What to expect

The PDE5 drugs have decades of randomized-trial support for erectile function, and daily tadalafil is also FDA-approved for urinary symptoms of an enlarged prostate (BPH) — a genuine bonus for many men. PT-141 is FDA-approved for low desire in premenopausal women and used off-label in men. Tadalafil suits daily dosing; sildenafil is as-needed.

Risks & considerations

PDE5 inhibitors commonly cause headache, flushing, congestion; the serious risk is a dangerous blood-pressure drop with nitrates or certain BP drugs, plus rare priapism (prolonged erection needing emergency care) or sudden vision/hearing changes. PT-141 commonly causes nausea, can transiently raise blood pressure, and with repeated use may darken skin/gums. A provider medication review is essential.
